Faith can help you with all Sacramental records questions and requests, but the information below may give you some help first.

If you are a parish representative requesting a certificate, please call Faith at the number below or click here to email her.

If you were baptized at St. Elizabeth Ann Seton Church, Guardian Angels, or St. Boniface, your baptismal record and subsequent sacramental records (First Communion, Confirmation and Marriage (if applicable)) should be here.

If you were NOT baptized at one of the three churches mentioned above, contact the church where you were baptized for your certificate. Even if you received other sacraments here, the baptism certificate will list ALL sacraments received.

Genealogy requests are handled through the Department of Archives and Records rather than by individual parishes.
